INFORMATION STATEMENT FORM
Title of the project
Name of organisation
We are gathering feedback as part of a larger research project to find out what kinds of services and support work to support families with young children best in which locations. Taking part is completely voluntary and you will not be disadvantaged if you choose not to.
What is the feedback for?
This program is part the Commonwealth Department of Family Community Services Stronger Families Fund program. What we learn will help the Government with developing family policies and programs to better meet the needs of children and families.
Is there likely to be a benefit to me or to others?
You may find it interesting to share your experiences, thoughts and knowledge and to hear and learn from others. What we learn may help improve programs and help other people in the future.
What is involved?
You may be involved in a group discussion, questionnaire or listening circle. The discussions may be tape recorded and written up, but you and your comments will be anonymous. You should respect the privacy of other group members by not talking outside of the group. Your involvement is entirely voluntary. You will be asked to sign a consent form to show you understand this.
Who are the researchers?
The project team (listed below) will oversee the project. The discussion group will be led by (the project workers name).
What questions will be asked?
The project worker will ask about your experience of taking part in this program, what you liked about it, what you didnt like, and what things you would change.
What are the possible risks?
You will be free not to answer any question asked, and to leave the group should it be distressing. The project worker will support you and arrange other help if needed.
Can I stop if I change my mind?
Yes. You can leave at any time and ask your name and information to be removed from the research records. There will be no disadvantage to you if you withdraw.
What will be done to make sure the information I give you is confidential?
The consent form, tapes and any written information will all be given code numbers and your name on the consent form will be kept separately in a locked filing cabinet. Project information stored on computer will be password protected. When the project is completed, tapes will be erased and computer files deleted. Written copies will be stored in locked filing cabinets for five years, then destroyed.
What will happen to the results?
The results of this program will be reported on the SFLEX database at A.I.F.S that will only be accessible to SFLEX and other SFF projects, so we can learn from each others experiences. They will also be reported by SFLEX to the Department of Family and Community Services, and may appear in articles in the media, A.I.F.S. publications and journals. Generally, only summaries of what everyone says will be used. You will not be identified in any of the research results.
Can I get a copy of the results?
When the report is available to read you will be notified if you let your project worker know you are interested.
What if I want more information?
If you have any questions, you can ask a project worker or the project manager (name and telephone number). We hope you will agree to take part, and look forward to seeing you in a group.
