The pages that follow include consent forms that you may find useful to copy and give to project participants. These are only necessary if people are involved in action research evaluation activities. They are not needed if people are participating in normal project activities like play groups, community picnics or parent education courses.

There is an Information Statement Form that explains how your project will work ethically. You can copy this, add to it, or delete bits and give it to the people who are involved in the ways listed above.

There is a Consent to Participate Form for people to sign to say they are prepared to participate in the action research evaluation part of the project

There is one more form (and then that’s all) that people must sign if you are going to make public use of any of the information they give you e.g. of written information, photos, videos etc. - Consent to Use Personal Information, Photos or Videos Form
